  1. Use this form to request the Mayor's or a Council Members participation in your event or function.
    Please note that requests must be submitted two weeks in advance.

    The below form is not associated with requesting the Mayor or Council Members to National Night Out parties. To register your neighborhood for National Night Out, which will automatically submit a request for the Mayor and City Council Members to attend your party, please visit
